Online social networking resources have become one of the most accessible mediums for professional reputation building for audiences inside and outside of academia. This column briefly discusses a selection of the many social network services that medical librarians commonly use and some ways in which they can form a component of the professional per-sonas we present to our online service and peer communities.

Objectives/Hypothesis: To assess compliance with Accreditation Council for Graduation Medical Education standards of professionalism among otolaryngology residency applicants with publicly searchable Facebook profiles. Study Design: Case series. Methods: Applicants to an otolaryngology residency program were searched on the Facebook website. Multiple matches were narrowed by available information until the correct individual was identified. Searches were performed in a manner that would not allow access to restricted information, thus showing only what would be accessible publicly. Four reviewers evaluated all profiles and generated a professionalism score. Scores were collated with National Resident Matching Program data and match results to identify significant associations. Results: Of the 234 applicants to our institution, 119 had Facebook profiles. Of these, 85 profiles contained information beyond simple demographics. Eleven percent of applicant profiles contained pictures or text that at least one reviewer felt could be perceived as unprofessional. There were six profiles with content considered to be concerning and one profile that contained clear violations of professional boundaries. Professionalism score did not correlate with whether the applicant matched, was interviewed, or received a place on the rank list. Age, gender, marital status, and Step 1 score did not correlate with a lower professionalism score. Our series included 61% of all otolaryngology applicants. Conclusions: Publicly available Facebook profiles with questionable content existed for 11% of otolaryngology applicants in this series. This finding did not affect applicants' match outcomes, nor was it predicted by any data available on the applicants' applications.

In this paper, we examine the consequences of social networking for an individual's morality, arguing that the content and approach of networking have different implications for how a person feels during the development and maintenance of social ties. We focus in particular on professional-instrumental networking: the purposeful creation of social ties in support of task and professional goals. Unlike personal networking in pursuit of emotional support or friendship, and unlike social ties that emerge spontaneously, instrumental networking in pursuit of professional goals can impinge on an individual's moral purity-a psychological state that results from viewing the self as clean from a moral standpoint-and thus make an individual feel dirty. We theorize that such feelings of dirtiness decrease the frequency of instrumental networking and, as a result, work performance. We conducted four studies using both field and laboratory data from different populations to investigate the psychological consequences of networking behaviors. Two experiments provide support for a causal relationship between instrumental networking for professional goals, feeling dirty, and need for cleansing. A survey study of lawyers in a large North American business law firm offers correlational evidence that professionals who experience feelings of dirtiness from instrumental networking, relative to those who do not, tend to engage in it less frequently and have lower job performance. With regard to sources of variability in dirtiness from instrumental networking for professional goals, we document that when those who engage in such networking have high versus low power, they experience fewer feelings of dirtiness. An additional experimental study constructively replicates this finding.

Professional networking involves a series of goal-directed interpersonal interactions that build and maintain professional relationships and include the exchange of work and career-benefiting resources. Despite the utility of professional networking, many people eschew the activity, which has spurred interest in the question: "why do people network?" Drawing from psychological theories of personality and behavioral prediction, we offer a theoretical basis for Professional Networking Motives (PNMs) that accounts for prior explanations for why people network. We verify and elaborate upon our initial theoretical conceptualization of PNMs by drawing from a series of semi-structured interviews of industrial and organizational psychologists based in the United States to identify the domain-specific content of PNMs (Study 1). We further posit that "why" people network (i.e., their PNMs) has implications for "how" they network (i.e., networking behaviors) and their career success. In Study 2, we develop and validate a measure of PNMs based upon occupationally diverse samples based in the United States, and we use a time-separated research design to evaluate whether PNMs differentially predict networking behaviors and career success. We illustrate the value of understanding people's motives for professional networking, as certain PNMs are more important than others for promoting different types of networking behaviors and forms of career success. We conclude by discussing the theoretical and practical implications of our study and offer suggestions for future research to deepen the un-derstanding of professional networking and PNMs.

Twitter acts as an information gateway as it provides a place where professionals network and share their knowledge. Twitter has increasingly influenced the way people use and share information. However, limited research demonstrates IT professionals' information experience on Twitter impacts the way they use it for professional purposes. The study aimed to understand how such information experiences impact on the way IT professionals use Twitter for professional purposes. Eleven IT professionals were recruited for this study to understand the participants' information experience through their own individual perspective, with the data analysed using constructive grounded theory. This study revealed that IT professionals' information experience plays a vital role in creating professional networking and knowledge sharing in online spaces. These lived experiences influence the way IT professionals use Twitter for professional purposes. Thus, the findings of this study contribute to theoretical perspectives in the understanding of information experience perspectives within Twitter, along with a foundational understanding of the ways in which microblogging is used for professional purposes. The findings can help organisations understand and provide for this emerging channel of professional information sharing for its staff and stakeholders.

Nursing faculty members at one midwestern college provided a professional development opportunity for baccalaureate nursing students by organizing a trip to a nursing association conference (ie, the AORN Global Surgical Conference & Expo). Preconference and postconference survey results showed a statistically significant increase in sense-of-belonging scores; and the eta-squared statistic (0.14) indicated a large effect size, suggesting the students' conference attendance enhanced their sense of belonging to the profession. A Pearson's correlation analysis revealed a strong, positive correlation between a student's intention to join a professional organization and his or her belief that membership in a professional organization was important (r = 0.82, n = 26, P <.001). This type of professional experience may improve students' perception of nursing professionalism, sense of belonging, intention to join a professional organization, and confidence in nursing as a career.

A significant number of patients with schizophrenia do not respond adequately to an initial antipsychotic trial. As first step within a treatment algorithm for therapy-refractory schizophrenia 'pseudoresistance' should be ruled out (eg, re-evaluation of the diagnosis, comorbidities, compliance and adherence in terms of medication intake, adequate dose and treatment duration, and achievement of sufficient plasma levels). In case of treatment resistance, two strategies that are often used in clinical routine care contain dose increase of the current administered antipsychotic drug (dose escalation, high-dose treatment) and switch to another, new antipsychotic. Although the response rates for both options are generally rather low, we see from the evidence-based perspective a slight advantage of the switching strategy (preferably to an antipsychotic with a different receptor-binding profile) compared to a high-dose treatment. After treatment failures with at least two different antipsychotic drugs, a monotherapy with clozapine is considered to be the treatment option of first choice. At present, pharmacological combination and augmentation strategies cannot be regarded as a generally recommendable evidence-based treatment method. Antipsychotic monotherapy should be preferably sought. In case of combination treatment, it appears more appropriate to combine preferentially two antipsychotics with different receptor-binding profiles. Augmentation of antipsychotics with other agents should be used primarily to treat specific target symptoms.
